Planned Parenthood Northern California (PPNorCal)is seeking a Temporary Bilingual (Spanish) Community Health Worker to support our Solano County community, working at our Fairfield and Vallejo Health Centers.

Work Schedule
Monday: 8:30 AM to 5:00 PM 
Tuesday: 8:30 AM to 5:00 PM 
Wednesday: 8:30 AM to 5:00 PM
Thursday: 8:30 AM to 5:00 PM 
Friday: 8:30 AM to 5:00 PM 

* flexible schedule, including evenings and weekends, as business needs require

SUMMARY

The Community Health Worker (CHW) serves as a trusted liaison between Planned Parenthood Northern California (PPNorCal), patients, and the communities we serve. The CHW improves access to healthcare, reproductive health services, and community resources by providing culturally responsive patient resource navigation, health education, outreach, advocacy, and care coordination.

Working collaboratively with health center staff and community partners, the CHW addresses social determinants of health (SDOH), promotes health equity, and supports PPNorCal's mission of providing inclusive, high-quality reproductive healthcare. The CHW serves as an integrated member of the care team while fostering meaningful relationships that improve health outcomes and expand access to care.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The essential duties listed below are representative of the primary responsibilities of this position and are not intended to be an exhaustive list. Additional duties may be assigned based on operational needs.

Patient Support Services

• Build trusting relationships with patients through culturally responsive, trauma-informed, and patient-centered care utilizing active listening and motivational interviewing techniques 
• Assess patient needs and assist with identifying and reducing barriers to healthcare access 
• Connect patients with internal and external resources, including food, housing, transportation, legal services, behavioral health, care coordination, telehealth, and other community supports 
• Assist patients with MyChart enrollment, scheduling follow-up appointments, insurance verification, enrollment applications, and access to healthcare services
• Maintain current knowledge of Medi-Cal, Family PACT, and other health coverage programs to assist patients with eligibility, enrollment, renewal, and ongoing access to benefits 
• Provide ongoing follow-up and support to patients as appropriate 

Community Outreach & Partnerships

• Develop, maintain, and strengthen collaborative relationships with community-based organizations (CBOs), schools, healthcare providers, advocacy organizations, and other community partners 
• Represent PPNorCal at community events, outreach activities, health fairs, coalition meetings, and educational forums 
• Identify partnership opportunities that expand access to reproductive health education and community resources 
• Support grassroots outreach and community engagement efforts that increase awareness of PPNorCal services and programs 
  
  

Health Education & Advocacy

• Deliver culturally and linguistically appropriate health education related to sexual and reproductive health, disease prevention, vaccinations, STI prevention, wellness, and available community resources, this may include bilingual presentations
• Encourage patient self-advocacy through education, resource connection, and health literacy 
• Promote health equity by addressing social determinants of health and advocating for equitable access to healthcare services 
  
  

Documentation, Reporting & Program Support

• Maintain accurate, timely, and confidential documentation in accordance with organizational policies, HIPAA requirements, and grant requirements 
• Track patient encounters, outreach activities, community partnerships, and program outcomes using assigned technology and reporting systems 
• Collect and document patient impact stories and program data to support grant reporting, quality improvement initiatives, and program evaluation 
• Assist with grant-related activities, including narrative development and program documentation, as assigned
  
  

Team Collaboration

• Function as an integrated member of the health center team by completing cross-training in designated clinical and operational workflows to support patient care and health center operations
• Collaborate with internal multidisciplinary teams – including Telehealth, Care Coordination, Patient Navigation, and Behavioral Health – to coordinate patient care and facilitate referrals
• Utilize electronic health records (EHR), patient management systems, Microsoft Office applications, and other assigned technology to perform job responsibilities effectively 
• Participate in organizational initiatives supporting Diversity, Equity, Inclusion, Belonging, and health equity 
• Perform other duties as assigned
  
  

MINIMUM QUALIFICATIONS

• High School Diploma or equivalent required 
• Associate's in Public Health, Social Work, Health Education, Human Services, or a related field preferred, or an equivalent combination of education and experience 
• Experience in community outreach, public health, healthcare, patient navigation, social services, or a related field preferred 
• Demonstrated knowledge of or experience serving communities experiencing health disparities and barriers to care 
• Ability to build rapport and establish trusting relationships with diverse populations 
• Demonstrated ability to work effectively with individuals from diverse cultural, socioeconomic, linguistic, gender identity, sexual orientation, disability, and lived-experience backgrounds 
• Excellent interpersonal, communication, and active listening skills 
• Ability to maintain confidentiality and exercise sound judgment 
• Ability to work independently while collaborating effectively as part of a multidisciplinary healthcare team 
• Bilingual English/Spanish proficiency may be required depending on the assigned health center or program needs 
• Ability to travel throughout the assigned service area to health centers, community locations, and outreach events as needed to perform the essential functions of the position 
• Ability to work a flexible schedule, including evenings and weekends, as business needs require

PHYSICAL DEMANDS

The physical demands described are representative of those required to successfully perform the essential functions of this position. Reasonable accommodations will be provided in accordance with applicable law. This position requires frequent sitting, standing, walking, speaking, hearing, reaching, and use of hands to operate office equipment and technology. Occasional lifting or carrying of up to 25 pounds may be required.

WORK ENVIRONMENT

Work is performed in health centers, community locations, outreach settings, and office environments. The position requires regular interaction with patients, community members, partner organizations, and multidisciplinary healthcare teams. The noise level is generally moderate.

Pay Range:  The wage scale reflects the compensation that a new employee can expect to receive if offered employment. The wage scale for this position is $27.55-$32.92 per hour, based on relevant professional years of experience.
